Professional Referrals
Patients can be referred to the service directly via www.gethealthyrotherham.co.uk

Professionals can complete a web based referral form that will ask for basic details regarding the patient such as name, date of birth, postcode, contact phone number etc.

All referral pathways into the service are secure.
Upon receipt of a referral, Get Healthy Rotherham staff will carry out a holistic lifestyle assessment of the patient and support them throughout a structure behaviour change intervention appropriate to their circumstances.

Get Healthy Rotherham is commissioned by Rotherham Borough Council and provided by Parkwood Healthcare.
